Pancreatic Fluid Collection Drainage
Pancreatic fluid collection drainage is a minimally invasive procedure performed to remove accumulated fluid around the pancreas. These collections often develop as a result of acute or chronic pancreatitis, trauma, or pancreatic duct leaks, and can lead to discomfort, infection, or compression of nearby organs if left untreated.
What are pancreatic fluid collections?
Pancreatic fluid collections include:
- Pseudocysts – fluid-filled sacs without solid material
- Walled-off necrosis (WON) – collections containing fluid and dead tissue
- Abscesses – infected fluid collections
These may cause abdominal pain, bloating, fever, nausea, vomiting, or difficulty eating.
Why is drainage needed?
Drainage is recommended when:
- The fluid collection is large or growing
- Symptoms like pain, vomiting, or infection occur
- There's a risk of rupture or obstruction of nearby organs
- There's infection or concern for pancreatic duct disruption
How is drainage performed?
The most effective and commonly used method is Endoscopic Ultrasound (EUS)-guided drainage:
- A flexible endoscope with an ultrasound probe is passed through the mouth to locate the fluid collection.
- Under real-time imaging, a small opening is created from the stomach or intestine into the fluid collection.
- A stent (plastic or metal) is placed to allow continuous drainage of the fluid into the GI tract.
In some cases, additional procedures such as necrosectomy (removal of dead tissue) may be required.
Advantages of endoscopic drainage:
- Minimally invasive (no external incisions)
- Faster recovery compared to surgery
- Lower risk of complications
- Often done as a day-care or short-stay procedure
- Suitable for both pseudocysts and walled-off necrosis
Is it safe?
Yes, EUS-guided drainage is considered safe and effective, especially in expert hands. Minor risks include bleeding, infection, or stent migration, all of which are typically manageable.
Aftercare:
- Patients are usually monitored briefly in the hospital.
- A soft diet may be recommended initially.
- Follow-up imaging ensures proper resolution of the collection.
- Stents are usually removed after a few weeks to months, based on healing.
